You must read the following and follow the steps below:
 

1.  You must have received at least a "B" in my classes.  I can't recommend anyone who doesn't do at least that level of work.

2.  If you are a Health Science student, you must have completed both HSC 210 and 220 with me.

3.  Give me plenty of time.  It may take me a month to get to it.  Don't wait until the last minute.

4.  Make sure you fill out your portion of the recommendation form.  If I have to call you in because you have not done so it will only take longer.  This includes the Buckly amendment/confidentiality clause.  ALL forms have them somewhere.  You must check off your waiver of access.  If you do not waive your access to the forms, I won't complete them.  If you are uncomfortable waiving your access, get someone else to give you a recommendation.

5.  When you bring me your forms, bring also a personal resume'.  Let me know what other activities you have been involved in while at UM.  I can make your letter more personal with this information.

6.  Print this page and sign it and bring it with you when you drop off the forms.  This shows you have read the above.
